I, [Participant's Name], hereby acknowledge that I have voluntarily chosen to participate in the basketball runs organized by BOSS PHYSIO, with the exception of the free invitational session on April 3rd, 2024, held every Wednesday from 8:15pm to 10:00pm, officially commencing from April 10th, 2024, and concluding on June 12th, 2024.

I understand that participation in this activity involves inherent risks and dangers, including but not limited to physical injury, and I voluntarily assume all such risks.

In consideration of being allowed to participate in the basketball runs, I hereby waive, release, and discharge any and all claims, actions, causes of action, liabilities, and demands whatsoever, that I, my heirs, executors, administrators, or assigns may have against BOSS PHYSIO, its officers, directors, employees, agents, representatives, and volunteers, arising out of or in connection with my participation in the basketball runs.

I certify that I am physically fit and have no known medical conditions that would prevent me from safely participating in this activity. In the event of an injury or illness, I authorize the staff and volunteers of the basketball runs to administer first aid and seek medical treatment as deemed necessary.

I understand that the payment of $50 is required upfront for participation in the 10 weekly basketball runs from April 10th, 2024, to June 12th, 2024. I agree to pay this amount in full before the commencement of the first paid session on April 10th, 2024.

I acknowledge that this waiver and release of liability shall be binding on me and my heirs, executors, administrators, and assigns. I have read this waiver and fully understand its terms, and I voluntarily agree to its contents.
